The COVID-19 vaccination for 5-11 years old in Tagbilaran City is now on! Please prepare the following documents to be presented prior to the vaccination:

1. Documentary proof of your relation to the child. Birth or baptismal certificate is accepted;

2. Valid ID with picture of the child;

3. Valid ID with picture of the parent / guardian;

4. Medical certificate if the child has an underlying medical condition;

5. Face mask and Hand sanitizer;

In the absence of the parents, the authorized guardian should present the following:

– Special Power of Attorney (SPA) or authorization letter from the parents which are duly notarized or affidavit from the parents informing consent (witnessed by a public official (notary public) or barangay official

-Valid government ID or Barangay Certificate signed by the Punong Barangay
